Eye emergencies and additional testing

Eye emergencies and certain scheduled testing are billable to your medical insurance. In the event of eye trauma,swelling, irritation, painful redness, extreme light-sensitivity, flashers, or pain, call our office immediately to schedule emergency care 970-565-7195. Our doctors are able to remove foreign bodies such as metal,wood, or plant fibers.

Additional testing such as Visual Field, Intraocular Pressure, Retinal Photos, or other diagnostic testing may be billed to your medical insurance if medically necessary. Coverage for theses tests depends on your specific plan (the services may be covered at a certain percentage, or applied to your plan deductible.)

Vision Service Plan (VSP)

We are currently providers for VSP. Generally, these plans cover an annual eye exam with a specified copay (amounts vary;call us, or your plan administrator for details.) Most VSP plans also offer an allowance for either glasses or contact lenses. The amount of the materials allowance varies, our opticians and/or contact lens technicians can help you understand what your specific plan allows. Because, we are an in network provider, we are allowed to bill the insurance company directly.

Medicaid

Medicaid will cover a routine eye exam and basic glasses for all eligible patients under the age of 21.

Medicare

Medicare is considered a health insurance for patients who qualify for coverage. We are able to bill directly to Medicare. Medicare may cover additional testing as prescribed by your doctor (see "additional testing" above) but will not cover screenings.

Medicare will help cover the health portion of your exam if you have met your annual deductible. Medicare will  not cover any  prescription eye wear.

Triwest/Veteran's Choice patients

We are able to bill Veteran's Choice for an annual eye exam and possible followup testing. However, the VA will not cover the cost of glasses or contacts from our office. To be covered, vision correction must be ordered directly from the VA (Albuquerque,NM or Grand Junction,CO) and our staff will assist you in this process. If you wish to purchase glasses or contact lenses in out office, all veterans receive a 10% discount. Thank you for your service.

Out of Network plans

We are currently not a provider for Davis Vision, Eye Med, Blue View, and Spectera. However, we do have reimbursement forms below that are printable.
